DOMESTIC SUBSCRIBER DISCONNECT NOTICE

Any inquiry or complaint regarding your bill may be addressed to Southwest Public Power District, Billing Department, PO Box 289, Palisade NE 69040 or by calling 308-285-3295. A domestic subscriber has the right, prior to the disconnection date, to request a conference regarding any dispute over a proposed disconnection. Service will not be disconnected pending the conclusion of such conference.

A disconnection shall be postponed or prevented upon presentation of a duly licensed physician, physician’s assistant, or advanced practice registered nurse’s certificate, which shall certify that a domestic subscriber or resident within such subscriber’s household has an existing illness or handicap which would cause such subscriber or resident to suffer an immediate and
serious health hazard by the disconnection of the utility’s service to that household. Such certificate shall be filed with Southwest Public Power District within five days of receiving notice of disconnection, excluding holidays and weekends, and will prevent the disconnection of services for a period of at least thirty days from such filing. Only one postponement of
disconnection shall be required for each incidence of nonpayment of any past-due account.

A domestic subscriber may arrange with Southwest Public Power District for an installment payment plan.

A domestic subscriber who is a welfare recipient may qualify for assistance in payment of their utility bill and should contact their caseworker in that regard.

Office hours are 8:00-5:00 Central time, Monday through Friday.
